Testosterone replacement therapy (TRT), which aims to increase overall serum testosterone levels, is the cornerstone of treatment and is primarily administered via injection, oral, or topical means. The availability of oral testosterone undecanoate medications like Jatenzo and Tlando has helped reduce the risk of adverse reactions at the injection site that are typical of intramuscular testosterone injections. However, a key drawback of TRT is reduced spermatogenesis, and some alternative therapies for low T focus on stimulating gonadotropins to preserve fertility. Additionally, the FDA has cautioned against the use of TRT because of the possibility of increased cardiac risk.
